It's that time of year again — time to pull out your back to school checklist. From shopping for school supplies to filling out permissions forms, let's not forget the most important item on your list — annual school and sports physicals.

A yearly physical exam by your family’s pediatrician or family doctor is an important part of your child’s health care. While specific requirements vary by school and/or sports organization, school and sports physicals can help ensure that your child is healthy and physically prepared to join in activities. During the exam, your doctor will review the complete health history and ensure that your child is up-to-date with immunization and vaccine recommendations for their age.

Physical exams

State of Illinois school physical forms, dental forms and vision forms are available on the Illinois Department of Public Health Web site at the links below.

School physical exams/proof of immunizations are required when your child is entering kindergarten, 6th grade and 9th grade.   

Sports physical exams are required when your child will be participating in a sport at the middle or high school level. Sports physicals are valid for one year (365 days) and cannot expire at any time during the sport your child is involved. If you take your child for a sports physical during the summer, it will be valid for the duration of the school year.

Dental exams

School dental exams/waivers are required when your child enters kindergarten, 2nd and 6th grades.

Vision exams

Vision exams/waivers are required when your child enters kindergarten.
