Welcome to the new athenahealth appointment confirmation and pre-registration for your next visit.*
The convenience of confirming your appointments online and completing general paperwork before you get to the office is getting a makeover.
It’s a new look and feel with the same goal: confirm your appointment and verify/update your patient information ahead of time, so your visit can start right away when you arrive at the office.
During this transition, you may receive communications from both our previous and new systems. Please be assured that this overlap is temporary and will not occur in the future. Thank you for your understanding and patience as we implement these improvements.

Messaging Details
Patients will receive notifications via email, text and/or automated calls leading up to the appointment for them to digitally confirm their visit and complete any pre-registration items. The sections below show a breakdown of all notifications leading up to the visit. Please note that if a patient completes pre-registration, additional notifications requesting completion of pre-registration items will stop.
Pre-Registration Details

Patients are encouraged to complete pre-registration digitally before their appointment to save time at check-in. Through pre-registration, you can:
Review contact information and demographics
Review and add insurance on file
Review and sign any necessary forms and documents
Review and add current medications and pharmacies
Review social and health histories
Pay outstanding balances or visit copay
Patients can start pre-registration by clicking the link within the email or text notification. You will first verify your identify by receiving a text or call to the mobile phone number on record. Then you will confirm if you are the patient or if you are representing the patient.
You will then be directed to the enhanced self check-in landing page where you will see multiple steps that need completed. You can see at a glance what steps are "In Progress" and what "Needs Review."

Add contact information and demographics. Patients can review the following fields:

Preferred first name
Date of birth
Primary phone and type
Consent to receive automated calls
Consent to receive texts
Mailing address
Demographics:
Preferred language
Race
Marital status
Insurance
Review any insurance on file or upload new card images. If adding images of new insurance cards, please capture both the front and back of the card.
Visit Forms
If patient is due to review or sign any forms and documents, those will populate in this section. Springfield Clinic's Patient Consent and Terms of Agreement form is required annually.
Medications and Pharmacies
Review and add all current medications, including prescriptions, supplements and over-the-counter medications. Patients can also add up to five preferred pharmacies and set one as the primary.
Social History
Answer questions related to your social history to help your provider understand your needs.
Health Histories
Review surgical history, past medical history, family history and allergies.
