Is enrollment automatic or is there a waiting period
for insurance to begin?
Coverage is not automatic and there are forms that must be submitted
to the Human Resource Department to enroll in all insurance coverage.
Human Resources will provide these forms to eligible employees
a few weeks before insurance is effective. The start date for
insurance coverage is the first day of the month following 60
days of employment (90 days for Dental, Life and Long-Term Disability).
To be eligible, an employee must be scheduled a minimum of 24
or more hours per week.
If I did not enroll in insurance when I was hired, can I change
my mind later?
Springfield Clinic's health plan does not have open enrollment
and an employee may only make changes or add coverage after their
initial enrollment period (60 days) if they have a qualifying
status change. If an employee does have a qualifying status change,
he or she MUST contact the Human Resources Department within
90 days of the event in order to add/change coverage. Our dental
plan does have an open enrollment period every March that allows
employees to make changes if necessary.
What happens to my insurance when I leave the Clinic?
COBRA law allows you and your family members to continue health
coverage for up to 18 months, and possibly longer, under certain
circumstances. To continue this coverage, you must complete and
return the COBRA forms to Human Resources within 60 days of the
separation date. Human Resources will mail these forms to your
home address, once we have been notified of the separation.
