Medical Insurance

  • Administered by Health Alliance
  • Requires Primary Care Physician (Clinic Physician)
  • Must use in network services for best benefit
  • No steerage towards any specific hospital
  • In network services - $50 individual, $100 family deductible per year, 90% coverage (applies to most services)
  • Out of network services - $200 individual, $600 family deductible, 70% of UCR after deductible
  • $25 office visit co-pay (does not apply to preventive services)
  • Prescription Drugs
    • Generic $15 co-pay
    • Preferred Brand $25 co-pay
    • Non-Preferred Brand $35 co-pay
  • Eligibility - must work 24 hrs. per week

Long Term Disability

  • Administered by the Standard Insurance Co.
  • Must work 24 hours per week
  • Voluntary Plan - Employee pays premium if he/she elects coverage.
  • Eligibility - first day of month following 90 days of employment
  • Benefit Waiting Period - 90 days
  • Monthly Benefit - 50% of earnings
  • Maximum Monthly Benefit - $6,000<
  • Minimum Monthly Benefit - $100
  • Maximum Benefit Period - Age 65
  • Premium is Age Banded and ranges from .35 - 2.63 per $100 monthly pre-disability earnings.

Group Life Insurance

  • Must work 24 hours per week
  • 2 1/2 times annual salary to $75,000 maximum (non - exempt)
  • 2 1/2 times annual salary to $150,000 maximum (exempt)
  • Entire premium paid by Springfield Clinic
  • Coverage effective 1st day of month following 90 day initial employment period
  • Life insurance coverage levels are calculated and updated annually, as of December 31st, with new levels effective March 1 of the following year.

Voluntary Life Insurance

  • Must work 24 hours per week
  • Up to 5 times annual salary to $500,000 maximum
  • Guaranteed issue amounts:
    • $100,000 for employee
    • $30,000 for spouse
    • $10,000 for children
  • Premium paid by employee through payroll deduction
  • Coverage effective 1st day of month following 90 day initial employment period

Vacation

  • Springfield Clinic vacation time accrues based on hours worked
  • 1- 6 years (10 days)
  • 7-14 years (15 days)
  • 15+ years (20 days)

Sick Leave

  • Begin to accrue with the second 6 months of employment at the rate of one day per month. Following the first anniversary of employment, begins accruing at rate of 10 days per year.
  • Maximum accrual for full time employees is 60 days.
  • Pro-rated hours for part time employees
  • Buy back days in excess of 60 at 1/2 rate at end of each year.

Personal Days

  • Springfield Clinic personal time accrues based on hours worked.
  • Employees are eligible to accrue 16 hours per year, up to a maximum of 24 hours.

  • Must work minimum 24 hours per week

Retirement Plan

  • Must be 21 years of age and work 1000 hours per year
  • Entry dates - January 1st or July 1st, immediately following completion of years of service requirement.
  • 401(k) Contributions:
    • Eligible after one year of service
    • Can defer up to 50% of earnings
    • Springfield Clinic matches first 1%
  • Profit Sharing Contributions:
    • Eligible after two years of service
    • Springfield Clinic makes contribution to Plan based on annual earnings (currently 5.7% - subject to change)
  • Merrill Lynch is trustee and record keeper of plan which provides 28 investment options.
  • Self directed plan (employees must direct investment of funds)
  • 100% immediate vesting
  • Administrative cost of plan paid by Springfield Clinic.
