Long Term Disability

Group Life Insurance
- Must work 24 hours per week
- 2 1/2 times annual salary to $75,000 maximum (non - exempt)
- 2 1/2 times annual salary to $150,000 maximum (exempt)
- Entire premium paid by Springfield Clinic
- Coverage effective 1st day of month following 90 day initial
employment period
- Life insurance coverage levels are calculated and
updated annually, as of December 31st, with new levels effective March 1 of the following
year.
Voluntary Life Insurance
- Must work 24 hours per week
- Up to 5 times annual salary to $500,000 maximum
- Guaranteed issue amounts:
- $100,000 for employee
- $30,000 for spouse
- $10,000 for children
- Premium paid by employee through payroll deduction
- Coverage effective 1st day of month following 90 day initial
employment period

Vacation
- Springfield Clinic vacation time accrues based on hours
worked
- 1- 6 years (10 days)
- 7-14 years (15 days)
- 15+ years (20 days)

Sick Leave
- Begin to accrue with the second 6 months of employment
at the rate of one day per month. Following the first anniversary
of employment, begins accruing at rate of 10 days per year.
- Maximum
accrual for full time employees is 60 days.
- Pro-rated hours
for part time employees
- Buy back days in excess of 60 at
1/2 rate at end of each year.

- Springfield Clinic personal time accrues based on hours worked.
- Employees are eligible to accrue 16 hours per year, up
to a maximum of 24 hours.

Retirement Plan

- Profit Sharing Contributions:

- Springfield Clinic makes contribution to Plan
based on annual earnings (currently 5.7% - subject to change)

Lynch is trustee and record keeper of plan which provides
28 investment options.
- Self directed plan (employees must
direct investment of funds)
- 100% immediate vesting
- Administrative cost of plan paid
by Springfield Clinic.
