Must work 24 hours per week
Up to 5 times annual salary to $500,000 maximum
Guaranteed issue amounts:
$100,000 for employee
$30,000 for spouse
$10,000 for children
Premium paid by employee through payroll deduction
Deduction taken on first and second paycheck of the month
Coverage effective 1st day of the month following 90 day initial employement period
